8:45 AM to 6:00 PM Monday, Tuesday, Wednesday, & Thursday
8:45 AM to 1:00 PM Friday
DOCTOR REFERRALS
In the state of Michigan, a doctor's referral is required for
treatment by a physical therapist, and insurance companies do
require a physician's referral prior to authorization of services.
Please call us if you are not familiar with your insurance coverage
and we will be happy to check the requirements of your policy with
your provider. Our office staff is happy to provide assistance in
obtaining a referral if needed.

Comfortable clothing (t-shirt, shorts, sweats, etc…)

LENGTH OF APPOINTMENTS
The average first appointment is 60 minutes. Please arrive 15
minutes early in order to fill out paper work.
Subsequent appointment times will vary from 45 minutes to 1 hour.
INSURANCE
· Our
billing office will bill all insurance plans including Medicare.
· As a
convenience we verify insurance benefits prior to your return visit
· Please
be aware of your co-payment, you may pay with each visit, or our
billing department will bill you on a monthly basis.

We also offer private pay options to our patients. Please contact us
